Hunt Regional Medical Center at Greenville and Hunt Regional Community Hospital at Commerce patients may obtain their medical records with the proper authorization.

 

These include diagnostic imaging records and billing information.

Request Process

A signed authorization for release of protected health information is required to release medical records.

 

Due to the requirement for a signature on the authorization form, employees of Hunt Memorial Hospital District (HMHD) are not permitted to accept requests for medical records by e-mail.

 

The person signing the authorization form needs to include a copy of a valid photo identification card. Any of the following forms of identification are acceptable:

  • a driver’s license
  • a photo ID from work
  • a military ID
  • any other type of photo identification that includes the requesting party’s name

 

Additionally, if you are the patient’s court appointed legal guardian or the patient’s Power of Attorney you must provide photocopies of the documents establishing your right to sign the authorization form on the patient’s behalf.

 

Print and complete the authorization form, sign the form, attach a copy of photo identification, and mail to the HMHD location where you were treated (addresses are included below).

Copies of Texas State immunization records


Billing Information

A signed authorization for release of protected health information is required to release billing information. The authorization form and a copy of photo identification should be mailed to Patient Financial Services (address is included below).


Diagnostic Imaging Films

A signed authorization for release of protected health information is required to release Diagnostic Imaging films. The authorization form and a copy of photo identification should be mailed to Diagnostic Imaging (address is included below).

Fees

The Texas Legislature sets the fees that hospitals are permitted to charge for copies of medical records. This is found in S241.154(e) of the Health and Safety Code. The medical record fees at HMHD facilities will not exceed the rates permitted by the Texas State Legislature.

 

Texas Fees for copies of medical records in paper form:

  • 1-10 pages $38.31
  • 11-60 pages $1.28 per page
  • 61-400 pages $0.64 per page
  • More than 400 pages $0.33 per page

 

Texas Fees for copies of medical records on microfilm or other electronic medium:

  • 1-10 pages $57.48
  • Subsequent pages $1.28 per page

 

Other Charges:

  • Diagnostic Imaging Films $17.00 per sheet (for subpoenas only)
  • Affidavit $10.00
  • Written response to a set of deposition questions $12.00

 

Additionally, hospitals are permitted to charge the actual cost of mailing, shipping or otherwise delivering the provided copies.
